About Us

Florida Ear and Balance is dedicated to providing state of the art, compassionate care for all of our patients. Due to the uniqueness of our specialty and the complexity of our cases, your initial visit to our office may take anywhere from 30 minutes to 2 hours, depending on your individual needs. Some services may require additional visits. Please make the arrangements for your work schedules and transportation as needed.

HMO or Managed Care Insurance Plans: These plans require prior authorization: this is an insurance company mandated policy. We are not responsible for obtaining your authorization. If you do not have an authorization when you arrive for your appointment you will not be seen, and you will be rescheduled when an authorization is obtained.

Please call the day prior to your appointment to make sure that your authorization has been received or bring your written authorization with you at the time of your appointment.

Insurance

Our website at "links" provides the necessary new patient information forms. Please print and complete the forms and bring with you; medical records that may be pertinent including hearing tests, M.R.I., C.T scans or other diagnostic tests. Bring the completed forms along with your insurance card and picture identification.
